Emerald Health Services is hiring a Travel RN - Case Management


Shift: Days / 8:30a-5p, 8x5, 40 hrs per weeks
Length: 13 Weeks


Requirements:

  • CA RN License
  • 2 years experience
  • BLS
  • Experience in a care management role preferred
  • BSN preferred
1012 RN - Case Mgmt 0830 to 1700 40 hours per week Provides excellent patient care by assisting in the collaboration development implementation revision and reporting of the case management program. Acts as a liaison between the patient family nurse physicians multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Coordinates a continuum of care for a defined population of patients from pre-admission through post discharge. Assures that patients and families proceed efficiently through the course of hospitalization. Function Clinical Support Family Care Management Level Registered Nurse (RN) FLSA Non-Minimum Education Bachelor's Degree (BSN) Preferred Minimum Work Experience Experience in a care management role Preferred Required Licenses Registered Nurses Registered Nurse (RN) licensure in state of practice Required Case Manager Case management certification Preferred Other Refer to the Required Licenses and Certifications list for additional job and/or unit specific requirements.
