Local - $79/hr OT: $118.5/hr ($3160/week) 

Travel - $94/hr ($37.8 nontaxed + $56.2 taxed) OT: $114.3/hr ($3760/week)

Facility Information

Must have:

-Active NY license at time of submission

-Must have BSN.

-NY PRI certification a plus but not required.

-Must be fully vaccinated against covid and provide proof of vaccination at time of submission.

-Discharge planning in hospital setting ADULT inpatient medical units including assessment, identification of barriers to discharge, escalation, referrals to post discharge facilities and vendors (e.g Homecare, Rehab),engagement of patient and caregiver in the plan, communication and collaboration with multidisciplinary team. Clinical Reviews to payors (use of MCG guidelines) as required.

-Expected to be able to jump in quickly to support discharge planning.

-Experience in inpatient hospital setting medical units preferred.

-System used: ACM/CarePort. EMR -Meditech.

-Orientation: 4 hr Fast Track training on system – Careport ACM, discharge planning concepts, Self-study modules with preceptor on unit.

-Professional attire; No Scrubs
