The Case Management team works to collaborate with the patients, families, providers and clinical staff to facilitate safe and timely patient discharges and promote quality and cost effective outcomes. The Case Management team is staffed by registered professional nurses and social workers to support patient needs.
Qualifications: The RN-Case Manager is required to hold a current New York State Registration and be certified in BLS/able to be recertified. A minimum of 1.5 years recent experience (within past 2 years) in an acute care facility of similar size and level as Hospital. Travelers will only be considered if experienced in hospitals of equal or greater size or trauma centers. Experience with an EPIC Electronic Medical Records is preferred. Candidates must possess excellent interpersonal and communication skills. Candidates must not have had professional license or certification suspended or investigated and not have been named as a defendant in a professional liability claim. Candidates must have verification of legal right to work in U.S. and successfully pass pre-employment health clearance, drug screen and criminal background check
