Job Description

Position Responsibilities:

Responsible for providing nursing care through all phases of care taking into consideration age and developmental needs. Collaborate and support plan of care as determined by the physician and healthcare team; treat patients accordingly as approved by the physician. Performs periodic re-evaluation of the patient as necessary and makes adjustments in the patients treatment program. Provide treatment to patients per protocols, utilize OASIS assessment and documentation. Perform on-going periodic system assessment and data collection in a systematic manner, focusing on: psychological, psychosocial, cultural spiritual and cognitive status; nutrition; pain; patient/family education; family involvement and patient advocacy; patient and patients equipment needs Facilitates timely discharge based on goals and in accordance with policies and procedures Educates and instructs the patient, patients family and other staff as required. Counsels the patient and family in meeting nursing and related needs. Supervises and teaches LPNs, HHAs as required for patient care according to law and regulations. Provides nursing care through all phases of care with consideration of age and developmental needs. Documents all care, intervention and care coordination per law, regulation and agency policy. Notifies patients physician and responsible staff of significant changes in a timely manner. Coordinates all services provided; consults with other health professionals as needed. Effectively plans, organizes, sets priorities and completes work assignments with minimum direct supervision per policies; within established time frames.

Required Education & Experience: Graduate from an accredited school of nursing; BSN preferred Current license to practice as a Registered Nurse in Colorado. A minimum of two (2) years experience as a Registered Nurse Previous Home Health experience is preferred. Clinical expertise in critical thinking and physical assessment of both the adult and children. BLS certification required. Notice: All licensure and certification must be obtained according to the current Licensure / Certification Policy and Procedure.

Skills and Special Abilities: Ability to function independently in the home environment. Sound judgment in decision-making; can react quickly and with confidence in a crisis or emergency situation. Recognition of own limitations in nursing practice and ability to seek out support and expertise of others. Compassionate, patient approach in working with chronically ill elderly. Adequate awareness of the dynamics of human behavior, especially the results of illness/disability on the family unit. An understanding of some of the special problems facing the elderly, the terminally ill or the acutely or chronically ill child and an ability to mobilize resources that will facilitate an optimal home environment. A non-judgmental, accepting attitude towards varying lifestyles and living situations. Knowledge of community resources and utilization of community services to augment services provided by Delta County Memorial Hospital. Able to care for patients of all ages. Ability to work effectively within role independently and with other team members. Ability to organize and complete work in a timely manner. Ability to read, write and effectively communicate in English.

Other: Must maintain an active drivers license. Must maintain a personal vehicle in operational order for travel in varying weather conditions at varying times of the day or night. Must maintain automobile/vehicle liability insurance coverage. Must maintain current proof of insurance and current drivers license in personnel file at all times. Must maintain an operational phone or equivalent means to be contacted except when on leave or vacation. J

Job Relationships: Directly responsible to the Clinical Manager. Interrelationships: works closely with physicians, hospital and Home Health Clinical staff, Home Health office staff, Home Health management staff, patients, families, and vendors.

Physical Demands Stooping, turning, bending, squatting, kneeling and the ability to lift up to 50 pounds; constant/repetitive standing; requires normal, correctable vision and hearing, and the ability to accurately discern color as necessary to perform job functions. Safety and Security: The individual actively promotes and personally observes safety and security procedures, and uses equipment and materials properly.
