Job Description

  • Responsible to team lead and Health Services Director Reports to team lead. Consults and collaborates with Medical Directors Works collaboratively with members of each team in Health Services and with other departments Provides direction to utilization management staff as appropriate
  • Complex case management is the coordination of care and services provided to members who have experienced a critical event or diagnosis that requires the extensive use of resources and who need help navigating the system to facilitate appropriate delivery of care and services.
  • The case manager completes a comprehensive assessment of the member's condition; determines available benefits and resources, and develop and implements a case management plan with performance goals, monitoring and follow-up.
  • For international business, the case manager coordinates care for international members who come to the United States for care.
  • This position may require on-call hours for after-hours care coordination needs.

Unit Description

This role works out of the Connected Care telephonic unit at a new, easily accessible, staff-only offsite support center. Many traveling consultants work out of this facility and the surrounding logistics are fantastic in terms of housing options, food, entertainment, and convenience for those coming to work from out of town.
