Job Description

Position Summary:

Utilizing a collaborative process, the care manager will assess, plan, implement, monitor, and evaluate the options and services required to meet an individual's health needs, using communication and available resources to promote quality, cost-effective outcomes. The care manager helps identify appropriate providers and services through the continuum of services while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain value for both the client and the reimbursement source. Position Qualifications

- Must have completed a program of education in nursing from an NLN approved institution. BSN required, MSN preferred. - 3 - 5 years prior experience in nursing. - Must be licensed in good standing as a Registered Professional Nurse. - Current BLS. - The successful candidate will be required to pass and maintain competencies for this position. - Knowledge of medical terminology. - Ability to work in-patient care activities

Private Housing is not provided

Initial Airfare to YKHC will be "reimbursed" upon candidate's arrival (This is only based on the average cost of a plane ticket, if the candidate brings additional items they will be responsible for the cost)

Cab vouchers will be provided based on the employee's proximity and distance to the hospital (this is done at the discretion of the Hiring Manager).

Immunization requirements from our Nurse Administrator:

- Hepatitis A - Hepatitis B - Varicella - Measles - Mumps - Rubella - T-Dap - Polio Or, Titers and/or the historical shot records for the candidate (Ex. Hep B is done usually from ages 1-4) If the candidate is not able to provide the shots listed above, they will not be accepted for the position.

Additionally, airfare is paid initially by the candidate and/or the agency and will be reimbursed upon the candidate's arrival here in Bethel, AK. Please let us know when this is done.

**Note: We require booking a SUNDAY NIGHT flight into Bethel as we have New Hire Orientation classes on Tuesday of each week. We DO NOT pick up travelers who fly in on Friday, Saturday or Monday.**
