Physical Therapist

1. General job duties: PT works under supervision of their Clinical Supervisor and the LTC administrator. Response to organize and conduct medically prescribed PT programs to restore function and prevent disability following disease

2. Equipment: Optima Health, overhead gait trainer, treadmills, balance master, Nustep, Parallel bars, Hydrocullator, Tillt table, wound vacs, weights, ultrasound, e-stim, bone stimulators, game ready or similar cooling unit, Mechanical lifts, dressing supplies, visual feedback, mat tables

3. Skills:WY lic, PT cert, will consider new grad with documented SNF experience . Prefer SNF experience of RUG management

4. Lic: WY lic, BLS

5.Shift: M-F 40 huors/week, may be asked to work occasional weekends based on resident case load

6. Floating: No, SNF assignment only at Legacy

7. Bed Size: 160 bed SNF facility with approx. 35 on caseload currently for Therapy. This includes Med A, Med B, private insurance and self-pay

8. Dress code: Black scrubs

We are a SNF with a 20 bed short term rehab dedicated unit however rehab residents may be anywhere in the facility. This includes LTC, dementia unit and short term rehab.
