CASE MANAGER (RN) - Mt Auburn Hospital - Monday-Friday, weekends as needed Education: BSN, Current MA License Experience: 5 plus years in Hospital Case Management/Discharge planning RN Case Manager assigned by the nursing unit coordinates discharge planning and monitors quality assurance for hospital inpatient admissions. Leads multidisciplinary team to ensure a timely process of Care Coordination.
