Job Description

Position: RN Case Manager (2 Needed), Term: 13 weeks , Shift:  Monday-Friday, 8a-5p, with some late nights, no weekends (36 hours guaranteed)  Desired Start:  ASAP,  MINIMUM QUALIFICATIONS  Education, Credentials, Licenses:   -3.5 years of experience as an RN**  -Minimum of 1 year of experience in case management (preferably in outpatient setting, specifically Cardiology) BLS Certification.  We do a lot of telephone work with patients.  We talk to them when they call in with problems, triaging the issue and determining if its an emergency or if its something that can wait for the doctor to make recommendations from a treatment standpoint.Candidates must be self starters, detail oriented, thorough, team-oriented and must be able to work efficiently and effectively with the care team and physicians to ensure quality care and patient satisfaction.  EMR system is EPIC and scrub color is black.
