Weekday call approx. 1/week ;4:30pm to 8:00 Experience - Mental Health Job Title: Home Care Registered Nurse Department/Subsidiary: Home Care Effective Date: February 2024 Job Code: Y6035 JOB SUMMARY: Provides individualized nursing care/assessments at the home/residence of the client diagnosed with acute, chronic and complex conditions. ORGANIZATIONAL SUMMARY A. Reports to: Clinical Nurse Manager B. Supervises: Home Health Aides (HHA’s) Licensed Practical Nurses (LPN's). C. Key Working Relationships: Physician's, Clinical Nurse Manager, Director of Post-Acute Services, Clinical Leader, Home Care RN's, HHA's, Therapists, LPN's, Service Coordinators D. Primary Customer Groups: Client's, Families, Colleagues E. Age/Population of Patients Served: Neonatal Adolescent Geriatric Pediatric Adult X All Ages ESSENTIAL FUNCTIONS A. Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis. B. Provides individualized care to clients with complex care needs including pediatrics, geriatrics, and mental health patients. C. Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care. D. Demonstrates the ability to care for clients with a variety of diagnosis’ including but not limited to orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs. E. Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage. F. Delivers nursing care for the clients in their homes, utilizing appropriate available resources. G. Involves the client/caregiver in goal development and plan of care development. H. Involves the client/caregiver in developing and carrying out the plan of care. I. Educates client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning. Evaluates and documents client's physical and emotional reactions to treatments, care, and medications in the electronic medical record (EMR). J. Conferences with other disciplines appropriately to provide an interdisciplinary approach to meet all identified client care needs. K. Assesses existing and potential problem areas, collects data, and reports significant information to the Clinical Manager or Clinical Leader. L. Functions purposefully in and adjusts to emergency situations as they arise. M. Instructs and supervises the home health aide, LPN, or other services provided to the client in their home requiring an RN supervision. N. Submits and maintains accurate, current, and complete documentation according to policy on a timely basis. O. Performs other responsibilities as requested by leadership. . P. Maintains required department confidentiality and abides by HIPPA regulations. Q. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments. R. Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs. Communicates with and performs duties following the philosophy of MercyOne North Iowa Medical Center and the Sisters of Mercy reflecting the organization’s Mission, Values, and cultural beliefs. S. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment. T. Performs other responsibilities as requested by supervisor, manager or department head. ADDITIONAL FUNCTIONS A. None. POSITION QUALIFICATIONS A. Education [formal]: 1. Current state Registered Nurse License or permit from State Board of Nursing required. 2. Completion of ADN required BSN preferred. B. Experience: 1. Must have demonstrated competency in an acute patient care setting for at least one to two years. 2. Home Health, community health, or hospice experience preferred. 3. Previous experience with mental health patients preferred, but not required C. Special Skills and Competencies: 1. Current motor vehicle driving license, motor vehicle insurance and available to travel to client's residences required. 2. Demonstrate the ability to use critical thinking skills and work in an autonomous setting. 3. Computer and typing skills required. 4. Must be able to read, write and/or speak English. 5. Must be willing to drive in winter weather 6. Completes the following mandatory courses (check all that apply): X Must provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status. X Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines Completes Advanced Cardiac Life Support (ACLS) – American Heart Association within 6 months of hire and maintains current status. Completes Pediatric Advance Life Support (PALS) – American Heart Association within 6 months of hire and maintains current status. Completes Neonatal Resuscitation Program (NRP) – American Heart Association within 6 months of hire and maintains current status. Completes MANDT Training within 6 months of hire and maintains current status. Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status. Motor Vehicle: _ MercyOne Vehicle [select this for positions required to drive a MercyOne owned Vehicle] • If required to operate MercyOne vehicle, will need to have proof of and maintain valid driver's license on file with Human Resources. • Note any special licenses or requirements for MercyOne vehicle operation (i.e. CDL)
